Unsafe, added charges, poor nursing supervisor

We were appalled at the lack of follow through with requests, poor adherence to COVID Guidelines, nurses being dishonest to cover their tracks, poor food quality to name a few. [name removed] was wonderful, some of their aides are excellent but a few are also mean & terrible with the residents. Cost went up every 1-2 months . Dad was able to escape easily from memory care & staff did not even know he was gone.

Like PRISON

My dear 82 yr old friend is in this hellhole and they dont even have a diagnosis yet. They have LOCKED HIM IN HIS "room/cell" so he feels trapped in jail. The nurses are VERY RUDE. I call my friend daily to cheer him up and see how he is doing. A nurse whose name I had never heard called me yesterday and threatened to file a Police Report due to my daily check in calls to my friend. Here is what I am reporting: #1 I am not even in their state nor jurisdiction. #2 I care about my friend very very much. My love for him as a father figure is very established. He calls me family as do I call him. #3 I'm not after his money But I am trying to protect him. We have talked financials often. With my 8 years of experience as a Money Manager/ Stockbrocker at Smith Barney in Ventura, CA. I DONT NEED MONEY. I NEED My Friend TO BE SAFE AND HAPPY. His niece filed a complaint to his [profanity removed] NURSE which is entirely frivolous, unmerited & baseless. After 18 days of forced isolation he wants to go home.

Awful experience

Do not - I repeat - do not entrust the care of your loved one to this facility. The staff is inexperienced, poorly trained and we felt no compassion from them. Our loved one died in their care under questionable circumstances and not one person from their staff even acknowledged us when we came there the day of their passing and on the days we came to clean out the apartment. No one even made eye contact. The management appears to be in shambles and we were told much of the staff has left. The kitchen staff was constantly turned over and short staffed and the food was terrible. Don’t put yourself through the pain we’ve gone through.

High Rent

Gives discounted rent rates to move in and then large rent increases every 6 months. Mom had to scrub her own toilet in between cleanings. Carpet in building used to be cleaned at least 1-2 times a month - have not been cleaned in over 2 years. Will falsify medical symptoms to be transferred to ER and then refuse to take resident back.

Landmark's Reckless Conduct, and Defiant Response

On the evening of [date removed], in my mom’s room at Landmark, I witnessed the following: - Male aide (I’m required to omit names), tasked with administering her evening pills, falsely claimed that two of her meds, crucial to her survival, were cancelled earlier in day by her primary doctor. - Landmark’s director of health services initially agreed with aide, then was forced to admit the mistake but offered no explanation, apology or even a hint of empathy. - Worse yet, she initially refused to speak with me, promised to call my sister/p.o.a but then refused, and abruptly disconnected the phone. Amazingly, their reckless medication error was not the only one that day: - In morning, female aide mistakenly gave evening-only med to my mom, then as a correction, instructed male aide NOT to give med in evening. But he either didn’t receive her communication, or failed to see it, and had planned to erroneously give med a second time in day. - When I asked male aide to verify whether med was given in morning, he falsely claimed that NO meds were given to my mom. I e-mailed a detailed complaint that evening to Landmark’s executive director, and absent a response, a follow-up the next evening. - On morning of [date removed], in phone conversation, executive director promised to send a written response explaining why these reckless errors occurred, and how they will be safeguarded from happening again. - Only one hour after her promise, in a stunning reversal, executive director stated in e-mail that Landmark had already “completed” an “internal investigation” on [date removed], had reported results to my sister and would offer no further comment. - Landmark’s claim is provably false, as executive director told me by phone that she didn’t receive my complaint until morning of [date removed] – one day AFTER she claimed to have conducted review. If you choose this facility, I urge you to be vigilant in supervision of their conduct, and do not expect cooperation from management.

I would never recommend it.

I would never recommend it. I was with the independent and assisted all together, and i didn't like that as much. The people that do the work are very nice, but the management are the big problem. Their word is no good, I had to pay for the wellness check, and there were months when we didn't even get the five minute wellness check. You had to pay for your meals whether you ate there or not. They are much more concerned about their bottom line than the care and health of the residents.

Not what it started out as.

When we first put my relative here the place was new and showed great promise. Unfortunately they don't deliver on their promises or programs. This is clearly not a facility for those who suffer with the start of dementia or Alzheimer's. they are very restrictive and don't offer anything that keeps a persons mind stimulated. Cafeteria style meals that get more complaints than not. The apartment has not been cleaned on a weekly basis, nor has her bedding been changed for quite a while. When we complained about this they just said that they were understaffed. There are not many activities to keep the people stimulated. My relative was not even able to sit out on a secure patio for fresh air unless a family member was there to take them. Many residents complained to us when we were there to visit. When checking out the memory care units we saw residents watching Sesame Street! While we were moving our relative to a new facility many residents said "Good for you! The place you are going to can't be worse than this!" There was a large staff turnaround. Look very closely before you spend your hard earned money here. The facility is new and the rooms nice but they have to emphasize on the care given to residents more.

Find another choice for your loved one

I would not recommend All Saints to anyone looking for a place for their family member. There is only a handful of staff that were amazing. Other than that, it was a horrible experience. Many residents, including our own parent had medication errors given. Many times meds were not sent when away for a weekend, they never had the meds re ordered, and no accountability. Several residents would use the emergency call button and would not receive assistance for well over 45 minutes. In one case, a resident had to use the bathroom so bad, that after 30 min of no staff responding, ended up having another resident help her to the restroom instead. Staff chatted with our family on many occasions and would tell us that staff would only go to rooms of residents they liked. If they did not like them, they would ignore the call light or make excuses. They are short staffed at all times, and the aides do not have adequate time with the residents. Very high turnover in staff. The director is not customer service friendly and VERY unapproachable. Other staff have commented how much she is not respected and has been cold to our family many times. I have seen her act like the residents are a burden instead of part of the family. They do not have a low sodium meal plan there. The meals are horrible- very high turnover in cooks. They do not accommodate the wishes of the residents and provide fresh veggies or fruit. Food is cold, high in sodium. The activity director was AMAZING and the activities planned were lots of fun. That was the one perk and good at All saints. The community has heard bad things about All Saints and I would encourage you to check out other places before sending your loved one here. Beautiful building, just horrible staff and care for your loved one. It is hard to leave each night.

A disrespectful disaster

The Waters contributed to my family's unhappiness and my mom's misery during the final year of her life. Despite a few kind and caring staff members, the standouts were those who treated my mom disrespectfully, one of who was verbally abusive, and the head of nursing, who was unresponsive to requests for change and detailed complaints. In general, management exhibited a lack of professionalism and concern. The Waters is clearly not set up for elders with frequent care needs. (My mom was of sound mind, wheelchair-bound and incontinent.) Care visits promised (and paid for) every three to four hours ended up being 7 or 8 hours apart (not good when someone is incontinent). Staff would speak over mom mom when attending to her, complaining about their jobs or discussing other topics as though my mom was not there. They'd sometime speak in their native language rather than English around her. Food was decidedly second-rate, and lack of wheelchair-accessible apartments was also a big down side. Laundry was spotty (literally and figuratively; several items of clothing were ruined due to improper laundering). The facility might serve the needs of those who are largely independent, but if a resident's needs increased, the Waters is clearly not the place unless some massive staff and management overhauls have occurred since 2013.

Landmark's Cold (and Misleading) Response to Family in Need

Our mom and Landmark resident went to hospital couple of weeks ago – and was abruptly diagnosed with a terminal disease. Mindful that Landmark would offer no exception to its visitor ban, we scrambled to remove mom from facility and divide care-giving for her so that she could see her family. That meant a sudden leave from jobs in the midst of an economic collapse. I sent letter to Landmark seeking to appeal to their compassion and high ethics, asking for a credit for final month’s rent when they provided us no service, for our family that has been ravaged by illness and financial distress. After a delay, Landmark’s executive director told me she was working on “financial piece” with my mom’s power of attorney, suggesting a credit was imminent. In truth, no credit was even offered, and exec director callously mislead me so that she didn’t need to directly refuse my appeal for help.

Other facilities are significantly better for your loved ones.

If you are looking for an assisted living facility for a loved one, there are other newer, better managed and staffed facilities near by, which are also less expensive. My Mom was there for 3 years and had a horrible experience, if you do have to put a family member here, even temporarily, make sure to set up a nanny camera (that is now legal in Minnesota) in their room.

Looks can be deceiving!

I don't usually write reviews, but this one is important to me. We toured several senior facilities before we finally chose the Landmark of Fridley. What a beautiful place it was, and still is. I live in another state and it was important to me that someone I love so much was taken care of in the best way possible. I have siblings who visit and check on her as well, but being so far away, it was comforting that we picked a place like the Landmark. As it turned out, looks are deceiving, and all the promises were ways to get a family's trust. After talking to my siblings, I came back to Minnesota and we had a meeting with the new director, who just started and he too saw things that we complained about. He made notes and asked for a follow up meeting before I left the state. He began to make changes, but because of health issues, quit!! Again an administrative change, and it has been downhill ever since! There is NO respect given to the elder people there, especially as their needs become more. And the director seems too busy to talk to the family. My senior relative can't go outside without a family member present. She comes to the front desk to see if they have something for her to do, and they are getting tired of that. They called my sister, and she had to find another place that would pick her up and take her to their facility to do activities. I am not sure what they are being paid to do. She goes 2 times a week, and the Landmark would like her to go every day. No one doubts that she is ready for some sort of memory care, but not what they consider memory care. Our relative is high functioning and should be kept busy to maintain what she still has. Landmark has their memory care residents watching Sesame Street and playing with Tinker Toys. We just moved her into another facility. She is now in a memory care program for high functioning seniors. Hopefully they will keep her brain active and challenged. I know dementia is a fast, nasty disease, but I also know that mind stimulating activities and communication is key and also taping into what used to be important to them. (Like simple cooking activities for those who used to do lots of cooking, and art or craft classes for those who enjoy that). The new place where our relative lives has the right idea, and I was told they are encouraged to visit with other residents. She is also encouraged to walk and goes outside regularly. Shame on you, Landmark, for not getting to know your residents and for not not making them feel important. Also, my sisters had to come in and do cleaning which the Landmark was supposed to take care of. If you are understaffed, Landmark, get help. I used to get a calendar emailed to me so I could see what was being done. That stopped after the first director left. Run the other way - don't waste your time or risk your loved one's life.

The Landmark in Fridley: Incompetent, inappropriate, untrained high turnover staff. Miserable care, unsafe oversight. Food is terrible. Activities demeaning and childlike. The Landmark staff do not understand seniors or seniors with dementia. No one in charge of facilities/cleanliness. Unstable nursing processes.Don't be fooled by the beautiful entry way or their promises - both deceiving.

We moved our mom into The Waters of Minnehaha, Memory Care (Mpls) last spring with high hopes. Although the facility is nearly new, we were fooled by the “chandelier,” & our experience was shocking & unacceptable. Here, on a regular basis, we witnessed neglect & maltreatment to a vulnerable adult. With much communication requesting “their promised care,” requests went ignored & mom was uncared for & neglected for hours & hours on a regular basis, without being checked. A sample of life -few if any daily “cares” administered, no grooming, stench odor, no prompt/assist to bed clothes, left alone for hours pacing, not put to bed, curled on a loveseat all nite, blaring loud radio or TV, bright lights all night, no interaction by the selectively “mute” staff, as it is not in the job descript to talk to mom.AIDES don’t do their job, NURSES don’t do their job, DIR NURSES doesn’t do their job and MANAGEMENT doesn’t either. This is sadly the normal acceptable culture here. Many more grieving stories of this place, too numerous to mention. We moved our mom.
